Prior to completing this form, please make sure that your Supervisor has provided you with a copy of your shop. Supervisor must review and complete the Ellis Follow Up Form on all shopping reports with the associate, regardless of the score.

When completing the form, make sure all fields are completed.  Once completed and signed, click "save document" and then select "Next" to finalize.  Once finalized, your Supervisor will receive an email to countersign.  *Please note, the course is not considered completed until the Supervisor has signed.
